Application for CME Credit-Clinical Instruction Log

• The SDMS awards 4 CME credits per year to sonographers who teach students in a CAAHEP-accredited DMS program.
• This form needs to be filled out and submitted to the SDMS for appropriate CME credits (2 hours of teaching = 1 CME). 
• This form must be submitted back to the college in order for you (the clinical instructor) to receive your CME Certificate, which will be mailed to you.  You (the clinical instructor) should keep a copy of this certificate for proof of your ARDMS continuing medical education.
